What this policy covers

This policy explains how Fall Bloom LLC, doing business as Fall Bloom Mental Health, handles information you send through this website.

It does not cover the medical record kept for established patients. That record is handled under the practice notice of privacy practices, which you receive when you become a patient.

What is collected

The contact form collects the first name, last name, email address, phone number, and message you choose to enter. Nothing else on this site asks you for personal information.

The form also sends a short hidden tag identifying which practice the message belongs to, and a hidden field used to filter automated spam.

How it is used

Your message is used to reply to you and to arrange care if you decide to become a patient. It is not sold, rented, or shared for advertising.

Messages may be handled by the service that delivers them to the practice inbox. That service processes the message only to deliver it.

This form is not secure

Email and web forms are not confidential channels. Please do not send symptoms, diagnoses, medications, or other medical details through this form.

Sending a message does not create a patient relationship.

How long it is kept

Inquiries are kept only as long as needed to respond and to meet the practice record keeping obligations. If you become a patient, your clinical record is kept under Michigan and federal record retention rules.

Your choices

You can ask that your inquiry be deleted, and you can ask what has been kept. Use the contact form or the phone number listed once it is published.

Changes to this policy

If this policy changes, the updated version will be posted on this page.
