Home visits are available on a limited basis for geriatric patients whose mobility makes travel challenging. This is not a general replacement for office appointments or telehealth. If getting to the Clinton office is difficult, ask whether a visit at home might be appropriate for your situation. The purpose is a psychiatric conversation centered on the person receiving care. Claudia Fall, PMHNP-BC, can discuss concerns and goals directly with the patient and consider what kind of follow-up makes sense. What the Conversation Covers
A home visit can support the same evaluation and follow-up conversations you would have in an office: current symptoms, health history, sleep, daily routines, questions about medication, and goals for care. Medication is one possible tool, never a requirement. Decisions are made together based on symptoms, goals, and comfort level. If another therapist or specialist would better meet a need, Claudia provides referrals to trusted providers. Nothing about the home setting predetermines a diagnosis or a plan.
Preparing for the Visit
It may help to have an up-to-date medication list available and to choose a quiet space where conversation is possible. Think about the questions the patient most wants answered and any changes in sleep, mood, or daily functioning worth describing. There is no need to stage the home or prepare a formal report. The patient's own account remains central.
A family caregiver may join with the patient's permission. Before the appointment, talk about what the patient wants the caregiver to contribute and what they would prefer to discuss privately. When Another Format Fits Better
A secure telehealth visit may be a better fit when travel is difficult but a private video conversation is workable. An appointment at the Clinton office may be preferable when the patient can travel and wants a setting outside the home.
